- Click the blue box labeled “Login with PatientFi.”
- Enter your PatientFi credentials to access your account.

- Once logged in, go to the “Documents” or “Account Information” section of the portal.

- Important account disclosures
Step 3: View or Download Documents

- Click “View” to open the document in your browser.
